Contact the Chapter office nearest you for a fundraising kit that includes a third-party event proposal form (the Greater East Ohio Chapter must pre-approve all third-party events benefiting the Chapter). Download Third-Party Proposal Form. Submit your event report with the proceeds of your event (photos are always welcome)
