The Care Consultant provides intensive care consultation services to individuals who are living with Alzheimer's disease or other dementias and to their caregivers. Care consultant conducts home visits, psychosocial assessments, care plan development and coordination, education on the disease process and disease management, and referrals to services. Care Consultant represents the Association in a variety of professional, business, and public settings, including providing trainings and presentations.
Essential Job Functions
Assist individuals, family members, and caregivers through assessment, care coordination, problem-solving, education, planning, referral and follow-up.
Show results through evaluation and report outcomes of work.
Interact and coordinate with other health and human services providers.
Maintain expert knowledge on Alzheimer's disease, dementia, health/mental health care, geriatrics.
Maintain accurate records and statistics; meet deadlines; collect and maintain case load data; perform ongoing evaluations.
Provide clinical support to Helpline Specialist, and attend to visitors to the office, including those in a crisis.
Represent the Association to a variety of community meetings or events for outreach, engagement, collaboration, fundraising, and referral purposes, including trainings and presentations.

BS in social work or a related field plus at least 2 years of dementia experience and experience in performing assessments and care plans. Master's level Social Service/Mental Health professional with dementia-specific experience preferred. Training in the TCARE program a plus.
Knowledge of community resources.
Excellent verbal and written communication skills; highly organized.
Demonstrated experience using Microsoft Office Suite.
Must able and be willing to travel as needed to conduct home visits and Chapter business.
Evening or weekend work may be required on occasion.
