Principle Responsibilities:
Project Management: • Monitor and supervise project staff to ensure fidelity to the REACH II model and to ensure adherence to the policies and procedures of the Alzheimer's Association • Manage staffing of project caseloads to ensure the project achieves annual enrollment and outcome goals • Complete and maintain all required documentation and reports for program • Maintain good working relationships with project partners • Develop and implement ways to build awareness of the program Service Delivery: • Screen clients to determine appropriateness for participation in the REACH program • Perform functional assessment (Risk Appraisal Measure - RAM) with clients to determine appropriate individualized education and counseling for improving clients' coping skills • Conduct skill building sessions in clients' homes or in chapter office • Develop and maintain relationships with clients over a period of 6 to 9 months by providing regular contact with clients to monitor and ensure progress • Complete timely and accurate client notes, charts and other documentation related to the progress of program participants • Connect clients to additional chapter services • Provide I&R and care consultation services to Helpline clients and follow-up as needed

| Minimum Requirements |
Qualifications: • Must be credentialed as a LPC or a LCSW • At least two years supervisory experience • At least two years behavioral health experience preferably with older adults • At least one year experience working with clients in the home setting • Previous work experience using crisis management, conflict resolution and anger management • Knowledge and experience of family systems and of issues surrounding grief and loss • Basic knowledge of Alzheimer's disease and related forms of dementia • Experience working with evidence-based program(s) preferred • Excellent customer service, organization and time management skills • Working knowledge of community resources related to aging services • Proficiency in MS Office products, especially Word and Excel • Ability to physically bend, push, stand and walk. Must be able upon occasion to move or lift approximately 25 pounds
Working Conditions: • Normal office environment • Travel by car to home visits • Travel by car to attend meetings and make presentations within chapter territory
Equipment: Must be able to operate a car, computer workstation, fax machine, telephone, calculator, photocopier, audiovisual equipment, dolly and other equipment as needed.
Other: • Must hold a valid Texas driver's license and have a reliable vehicle available for work purposes • Must be willing to work on evenings and weekends occasionally as needed • Must accept other duties, as assigned |
