General Summary

The Care Consultation Coordinator position supports the mission of the Alzheimer's Association Cleveland Area Chapter to eliminate Alzheimer's disease through the advancement of research; to provide and enhance care and support and to reduce the risk of dementia through the promotion of brain health.  

Essential Job Functions

Supervise Care Consultation staff and oversee the administration of the Respite Reimbursement program.
•Provide telephone or in-person Care Consultation to individual caregivers or groups of family members; including assessment, education, care planning and referrals.
•Manage referrals and caseload in an effective and efficient manner.
•Acquire the necessary documentation for compliance with funding sources.
•Assist caregivers with setting up Emergency Respite care as needed.
•Provide individuals and families with regular telephone follow-up support consultations. 
•Maintain appropriate documentation and service statistics.
•Record documentation in a timely manner in the RODEO Data System.
•Assign Care Consultation referrals.
•Ensure Care Consultation materials are routinely updated.
•Develop and maintain policies and procedures for the Care Consultation program.
•Manage quality assurance activities including surveys and audits.

Minimum Requirements

Required:
Advanced degree with clinical licensure
Knowledge of Alzheimer's disease and other dementias
Two plus years clinical experience with individuals and groups
Supervisory experience
Knowledge of community resources in the field of aging
Strong organizational skills
Excellent communication skills
Advanced writing and computer skills: Word, Excel, Power Point, Outlook
Valid driver's license and car for travel within service area

TIME AND HOURS
40 hours/week
Monday-Friday
Some evenings are required
