Support Needed for Specialty License Plate

Help us raise awareness to find a cure!

Nearly fifty-one thousand people in Kansas right now have Alzheimer's disease and that number is growing. Alzheimer's license plates would raise awareness of the disease, which will become more prevalent as Kansas’ population ages.

If you haven't already done so, PLEASE SIGN UP TODAY!

The Alzheimer’s Association has received approval from the State of Kansas for an Alzheimer’s specialty license plate for Alzheimer’s disease awareness in Kansas.

Once we collect 500 applications from interested individuals, the state will issue the plates. Please help us meet this goal as soon as possible!

It’s easy!     
  

  • Fill out the application form and submit a check for $30.00 made out to the Alzheimer’s Association – Central and Western Kansas Chapter and send to: Alzheimer’s Association 1820 E. Douglas, Wichita, KS 67214. If we do not collect 500 applications by the deadline, the fee will be returned to you or you can designate it as a donation to the Alzheimer’s Association. 

           
  • Once the 500 commitments are secured, the Alzheimer’s Association will send you a certification statement for you to provide to your county treasurer’s office when the plates are available.  This certification will let them know that you have paid the required $30.00 royalty fee.

    We need your help to meet our goal! Spread the word to family, friends, neighbors and colleagues. Urge them to apply for this beautiful, purple license plate to help raise Alzheimer’s awareness and show support for those living with the disease as well as their caregivers
