Connections Plus is a new pilot program that seeks to foster support and social interaction for those individuals who are in the early stage of Alzheimer's disease or a related disorder.
This program is intended to empower people with early dementia through social connections with each other and to engage them to fully use and enjoy their strengths and capabilities.
Connections refer to individual matches between a person in the early stage with a Volunteer Mentor who is also in the early stage, to facilitate one-to-one informal social interaction and support.
Plus refers to scheduled group outings in the community for pairs of Connections participants and their Volunteer Mentors.
Connections Plus is for you if:
You are in the early stage of Alzheimer' disease or a related disorder.
You would like the opportunity to connect and share your experiences with someone who is also in the early stage.
You would like the opportunity to participate in social outings with others.
We are also recruiting Volunteer Mentors who are in early stage and who view their diagnosis not as a barrier, but as an opportunity to help others live their life to the fullest. All Volunteer Mentors will be provided with training and support to assist them in their one-to-one matches with others in the early stage.
Participants in the Connections Plus program will be asked to evaluate the success of the program to help shape the program to be even more beneficial for future participants.
