Tribute gifts are a special way to honor a loved one and support other caregivers struggling with dementia.  The Chapter greatly appreciates that so many people choose to remember their loved ones and the Alzheimer's Association in this way.  Gifts can also be made in honor of someone's wedding, anniversary, graduation or other special occasion!  For acknowledgements, please remember to include the following information:

Address Information of a Family Member to be Notified of the Gift
