Alzheimer’s Association License Plate

The four Alzheimer’s Association Chapters in Virginia are working together to have a special Alzheimer’s license plate.  License plate fees are $25 annually in addition to registration fees.  100% of the proceeds will benefit the delivery of programs and services for Virginians, their families and caregivers affected by Alzheimer’s disease.

Instructions: Send the form with a check made out to the Alzheimer’s Association Central and Western Virginia Chapter in the amount of $25.00. Credit card payment is also acceptable (Visa/MasterCard). In the “Plate Information” section of the form be sure to check the Vehicle License Plate box at the top and Special Interest box placing “Alzheimer's Association” in the blank line. Your license plates will be issued in 2010, soon after legislation is passed by the 2010 General Assembly.
