Care Consultant/Director of Family EducationThe Care Consultant serves as a key staff person in the delivery of patient and family services. In addition to assessing family needs, helping secure services to support care at home, providing one-on-one education and guidance, and coordinating or overseeing social engagement programs, the Care Consultant plays an important role in the Chapter’s education program, conducting caregiver training and in-services for home health agencies, residential care facilities and other groups.
