Care Consultant

Alzheimer’s Association, Maine Chapter seeks MSW or LCSW with geriatric case management, assessment and care plan experience; knowledge of  Alzheimer’s Disease and related issues a must; familiarity with community resources, systems and programs preferred; good communication, documentation, organization and computer skills required; in-state travel required.
