Alzheimer’s Association Memory Walk Incentive Sweepstakes - Alzheimer’s Association

To enter this Sweepstakes, you must agree to and abide by these Official Rules.

NO PURCHASE NECESSARY TO ENTER OR WIN.

1. Sponsor: Alzheimer’s Association, 225 N. Michigan Ave., 17th Floor, Chicago, IL 60601-7633.

2. Eligibility: “The Alzheimer’s Association Memory Walk Incentive Sweepstakes” ("Sweepstakes") is open only to legal U.S. residents 21 years of age or older at the time of entry. Any individuals (including but not limited to employees, consultants, independent contractors, and interns) who have, within the past six months, performed services for the Sponsor, any organizations responsible for sponsoring, fulfilling or administering the Sweepstakes or supplying the prizes, and any of their respective parent, subsidiary and affiliated companies, as well as the immediate family and household members of each such employee, are not eligible to participate or win. "Immediate family members" shall mean parents, step-parents, children, step-children, siblings, step-siblings, or spouses, regardless of where they live. "Household members" shall mean people who share the same residence at least three months a year, whether related or not. Potential winner may be required to provide proof of legal U.S. residency upon request prior to prize award. Void where prohibited or restricted by law.

3. How to Enter: The Sweepstakes begins at 12:01:00 am Eastern Time (“ET”) on 7/1/09 and ends at 11:59:59 pm ET on 11/30/09 (“Promotion Period”). Sponsor’s computer is the official clock for this promotion. Participants will receive one entry for every $500 in donations (up to 10 entries) received by the Association through 11/30/09.

TO ENTER BY REGISTERING FOR MEMORY WALK: When you become a Memory Walk Participant, you will receive one entry for every $500 in donations (up to 10 entries) you obtain from others that is received by Sponsor by November 30, 2009.

A Memory Walk Participant is defined as a person who has registered for Memory Walk either online at www.alz.org/memorywalk or via hard copy registration form from their local chapter. Registration means individuals sign up to participate in Memory Walk to walk as an individual or as part of a team, provide personal contact information to their Alzheimer’s Association chapter including name, address, phone and email, and are soliciting and collecting donations from secondary or third party sources to reach an individual Memory Walk fundraising goal.

TO ENTER BY MAIL (without registering): Participants may also enter by sending a letter, with proper postage, to Alzheimer’s Association with their name, address, and daytime phone number to: Alzheimer’s Association, P.O. Box 96011, Washington, D.C. 20090, c/o Memory Walk Sweepstakes. Limit one (1) mail-in entry per envelope and no more than ten (10) total entries per person by and/or all methods of entry.

Attempts to enter by the following methods are not valid and will not be counted: 1) funds donated or pledged to the Alzheimer’s Association during the promotional period, unrelated to Memory Walk; or 2) funds donated or pledged to another Memory Walk participant to support that person’s individual fundraising goal.

Registrations, mail-in entries generated by script, macro or other automated or mechanical means (including mechanically reproduced mail in entries) or by other means that subvert or intend to subvert the registration/entry process (as determined by Sponsor in its sole discretion) are void and may result in disqualification. Names collected will be based on data from the local chapter as of November 30, 2009. The Sponsor relies on its local chapters to provide updated, accurate and timely information. The Sponsor is not responsible for the actions or inactions of the local Alzheimer's Association chapters.

All registrations/entries become the property of Sponsor and will not be acknowledged or returned. Multiple entries in excess of this limitation or mass entry attempts will result in disqualification of all entries by such Participant. Entries must be manually key-stroked; use of automated entry devices or programs is prohibited.

4. Prizes, Approximate Retail Values ("ARV") and Odds:

Grand Prize (1): Round trip airfare for two (2) people from the airport closest to the Participant’s home with direct flights to Las Vegas and a three (3) day, two (2) night stay at Bally’s Las Vegas (double occupancy) plus a dinner for two at Bally’s Steakhouse (maximum amount covered is $150) and two tickets to the Jubliee Las Vegas show. All other expenses are the responsibility of the winner. Trip must be completed by December 31, 2010. All taxes on the prize are the responsibility of the winner.

Guest of the winner must be over the age of eighteen (18) years old or be the child or ward of the winner. Acceptance of the prize constitutes permission from the prize winner for the Sponsor to use the prize winner’s name, likeness, prize, biographical information, and state of residence for public relations, advertising, and promotional purposes online and in print, or in other media, in connection with the sweepstakes without additional compensation unless prohibited by law. By accepting prize, winner agrees that the Sponsoring Entities, their parents, affiliates, subsidiaries, distributors, retailers, and/or any of their respective shareholders, directors, officers, distributors, employees or promotion/advertising agencies will have no liability whatsoever for any injuries, losses, harm, damage, cost, or expense arising from or in connection with participation in this sweepstakes or any prize related activities, or the acceptance, possession, use and/or misuse of a prize. Winner and guest must execute liability and publicity releases.

Sponsor is not responsible for any lost, misdirected, delayed or illegible entries, nor for any incorrect or inaccurate information. Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of, entries. Sponsor is not responsible for any problems or technical malfunction of any telephone network or lines or, computer on-line systems, servers or providers, computer equipment or software, failure of e-mail on account of technical problems or traffic congestion on the Internet or at any website or combination thereof, including injury or damage to participants or to any person’s computer related to or resulting from participating or downloading materials in this sweepstakes. If, for any reason, the sweepstakes is not capable of running as planned, including infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of Sponsor which corrupt or affect the administration, security, fairness, integrity or proper conduct of this sweepstakes, Sponsor reserves the right at its sole discretion to cancel, terminate, modify or suspend the sweepstakes.

Participants further agree not to knowingly damage or cause interruption of the Sweepstakes and/or prevent others from participating in the Sweepstakes. CAUTION: ANY ATTEMPT TO DAMAGE ANY ONLINE SERVICE OR WEBSITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE SWEEPSTAKES VIOLATES CRIMINAL OR CIVIL LAWS. IF SUCH AN ATTEMPT IS MADE, SPONSOR MAY DISQUALIFY ANY PARTICIPANT MAKING SUCH ATTEMPT AND SEEK DAMAGES TO THE FULLEST EXTENT OF THE LAW.

Total Approximate Retail Value of all prizes awarded: $1700

No substitution or transfer of prizes except with Sponsor’s permission or where required by law. If for any reason announced prize/prize component becomes unavailable, Sponsor in its sole discretion may award substitute prize/prize component of equal or greater value. Limit one (1) prize per person. Odds of winning a prize depend on total number of eligible entries received during the Promotion Period.

5. Sweepstakes Random Drawing: The drawing will occur in January, 2010 through a random drawing of all eligible entries. Winners will be announced in February, 2010.

6. Grand Prize Winner Verification/Prize Award: The winning participant will first be contacted via mail by their local Alzheimer’s Association chapter. If the Participant’s reply is not received by Sponsor within two weeks, the Sponsor may elect to follow-up with a phone call and/or e-mail. Winner contact information will be based on participant registration information received from chapter. If no response is received within 30 days of initial mail contact, the sweepstakes winner’s rights will be revoked and a second drawing will occur to identify a winner.

7. Conditions and Limitations of Liability: By participating, entrants agree to be bound by and abide by these Official Rules and the decisions of Sponsor, which shall be final and binding in all respects. By participating in this Sweepstakes, entrants agree to release and hold harmless Sponsor, its parent, subsidiary, affiliated and successor companies, advertising and promotion agencies and prize suppliers, and each of their respective officers, directors, agents, representatives and employees, as well as each of their respective successors, representatives and assigns (collectively, the "Released Parties") from any and all actions, claims, injury, loss or damage arising in any manner, directly or indirectly, from participation in the Sweepstakes and/or acceptance or use of any prize. By entering, entrants also authorize the Released Parties to use their name, voice, likeness, photograph, prize information, biographical data and city and state of residence in programming, advertising, trade and/or promotional material or on a winner's list (if applicable) without compensation or notice, to the extent permitted by law. Sponsor is not obligated to use any of the above mentioned information or materials, but may do so and may edit such information or materials, at Sponsor's sole discretion, without further obligation or compensation. Force Majeure: The failure of the Released Parties to comply with any provision of these Official Rules due to an act of God, hurricane, war, fire, riot, earthquake, act of public enemies, actions of governmental authorities outside of the control of the Released Parties (excepting compliance with applicable codes and regulations) or other force majeure event will not be considered a breach of these Official Rules.

SPONSOR MAKES NO WARRANTY, REPRESENTATION, OR GUARANTEE, EXPRESS OR IMPLIED, IN FACT OR IN LAW, RELATIVE TO THE USE OF ANY PRIZE, INCLUDING, WITHOUT LIMITATION, ITS QUALITY, MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE.

8. Additional Information: For an additional copy of these Official Rules send a stamped, self-addressed envelope (residents of Vermont may omit return postage) postmarked on or before December 31, 2009 to: Alzheimer’s Association, Attn: Corporate Initiatives, 225 N. Michigan Ave., 17th floor, Chicago, IL 60601. For the winner’s name, available after the drawing in February, 2010, send a self-addressed, stamped envelope to the same address.
