Care Consultation is a service that can help you and your family develop a road map to navigate through the many thoughts, emotions, and questions you may have about memory loss and dementia. Care Consultation can be useful whether you or someone you love has just been diagnosed, or if you and your family are seeing things from different perspectives and perhaps disagreeing about the next steps to take.

Care Consultation services can help you build a plan around your future. You will receive valuable one-to-one assistance that will enable you to better understand the disease, manage care and make more informed decisions regarding services and treatments. Phone or in-person individual sessions are offered when it may be too difficult to involve others or if you feel alone in this process. The length of phone consultation varies, while in-person sessions are 60 to 90 minutes.

Care Consultations (both phone and in-person) will include:

  • Assessment of your current needs
  • Assistance with developing a plan
  • Assistance with finding resources and services
  • Problem-solving
  • Providing education and support
  • Providing ongoing support and follow-up
    Care Consultation can be a valuable source of support for those coping with the impact of dementia on their lives. This service is meant to address the unique and individual needs that will arise throughout the disease process.
