Introduction

 

If you or someone you know is worried or has questions about memory loss, a Care Consultation is a service to connect you with:
  • Information
  • Support
  • Care tips
  • Services
  • Health care and health care planning

Whether you are a person with memory loss, or someone who cares for someone else with memory loss, a Care Consultant will problem-solve with you to address a person's unique experience with Alzheimer's disease and related dementias. A Care Consultant will work with you by phone or by an in-office appointment. Please call 973.586.4300, to request a Care Consultation.

Family Care Consultation

Care Consultation is a service to assist the person with Alzheimer's disease or related dementias and/or their family care partners in planning for, and dealing with, all aspects of the impact of the illness. Individuals with dementia and their care partners will receive valuable one-on-one assistance that will enable them to better manage care and make more informed decisions regarding services and treatments.

Care Consultation may include any of the following:

  • Assessment of needs
  • Assistance with developing a care plan
  • Information about resources
  • Problem solving  
  • Education and support

Care Consultation meetings may help families with decision making about care options and provide a greater awareness and understanding of the changes that may occur with Alzheimer's disease.  Care Consultation can be a valuable source of support for those coping with the impact of dementia.

Care Consultation Options

Care Consultation can be delivered via telephone or by an office visit/in-person appointment.  This service is meant to address your individual needs concerning Alzheimer's disease.
