Family Care Specialist


REPORTS TO: Chief Program Officer
STATUS:  Exempt, Part-Time, 30 Hours per week  

POSITION SUMMARY

Under the direction of the Chief Program Officer, the Family Care Specialist is responsible for enhancing direct assistance and professional intervention to Alzheimer’s families and to foster cooperative relationships with the infrastructures of local community health and social service professionals. This is a part-time position, 30 hours per week, based in the Mountain View, California office.

ESSENTIAL JOB FUNCTIONS

PATIENT & FAMILY SERVICES (90%)

Casework with Alzheimer’s families
• Provide consultation to individuals and families (needs assessment, care planning, resource  referral, advocacy as needed, follow-up and documentation), in-office, by telephone and, if  needed by home visitation
• Coordinate with health and social service workers in assisting families

Support Group Programs
• Coordinate and facilitate Early Stage Support Group.

Helpline
• Serve as a resource to staff and Helpline volunteers
• Provide staffing coverage to Helpline as back up to Helpline Manager and Family Services Manager
• Develop appropriate materials as needed

MedicAlert + Safe Return Follow-up
• Respond to missing persons cases immediately
• Provide assistance after person is found   
    
Community Education (10%)
• Speak at Association public education programs, professional trainings and support groups
• Contribute to the Chapter Newsletter

Other duties assigned by Program Director

KNOWLEDGE, SKILL REQUIREMENTS and EXPERIENCE

• Masters in Social Work, Family Counseling, Gerontology or related health care field
• MSW or MFT Licensure Required
• 2 years experience in geriatric and/or Alzheimer’s care
• Record of speaking and writing on dementia and caregiver issues
• Care management and case documentation skills; ability to monitor and evaluate care plans, and refer to resources, ability to respond to emotional concerns of family members
• Public speaking skills
• Computer: word processing and database management
• Excellent verbal and written communication skills
• Supervisory experience desirable
• Experience working with diverse communities and demonstrate inclusion
• Bi-lingual skills desired

Self-starter, independent worker and willing to take initiative
Willingness to work some evenings and weekends
Willingness to travel in service area
Desire to help families cope with Alzheimer’s disease and related dementias.

Databse Administrator

 

POSITION SUMMARY

This position is primarily responsible for managing the Alzheimer’s Association program, development, and administrative databases.  These databases are principally Microsoft Access databases, but include support on third-party databases.  The Database Administrator is responsible for ensuring the efficiency and integrity of our databases.  Duties include: Troubleshooting connectivity and access problems with resolution, developing and following procedures to ensure compliance with organizational expectations, monitoring and upgrading applications as needed, and creating custom reports.  The Database Administrator is also expected to provide general IT support to staff as needed.

ESSENTIAL JOB FUNCTIONS

• Write queries and reports, create output files, and communicate with National MIS staff to ensure the databases in use meet the needs of the staff.
• Work closely with National office MIS staff and Chapter data entry staff and volunteers to ensure data integrity for program databases.
• Monitor, troubleshoot and upgrade program databases as needed

KNOWLEDGE, SKILL, EXPERIENCE AND EDUCATION REQUIREMENTS

• Bachelors degree with Microsoft and/or Database certifications preferred
• Proven experience in managing databases and understanding relational databases
• Demonstrated ability to convert, enhance, upgrade and troubleshoot databases
• Experience in a non-profit environment, supported primarily by private donors preferred
• Ability to lead a team of database administrators and data entry assistants
• Strong project management, organizational and analytical skills
• Excellent written and oral communication skills
• Strong problem-solving skills
