MedicAlert® + Alzheimer's Association Safe Return® is a nationwide wanderer's safety program created by the Alzheimer's Association that assists in the safe and timely return of individuals with Alzheimer's disease and related dementias who wander and become lost.  We are not a "wanderer's" safety program”, but a nationwide emergency response service for individuals with Alzheimer's or a related dementia who wander or have a medical emergency.

Only individuals with alzheimer's or related dementia are eligible for MedicAlert + Alzheimer's Association Safe Return.

In a move to significantly improve the safety of individuals with Alzheimer’s, the Alzheimer’s Association and the MedicAlert Foundation have created an alliance to bring you MedicAlert + Alzheimer’s Association Safe Return. MedicAlert + Safe Return offers you the best of both worlds:

• Assistance when a person wanders or is lost
• Access to vital medical information in the time of need

How the Program Works & Costs

When a person with dementia wanders or becomes lost, one call immediately activates a community support network to help reunite the lost person with his or her caregiver. When a person is found, a citizen or law official calls the toll-free 24-hour emergency response number on the identification product and the individual's family or caregivers are contacted. The nearest Alzheimer's Association office provides support during search and rescue efforts. In addition, should medical attention be required, access to a personal health record is immediately available.

Cost

The $62.00 fee includes:

• Individualized emblem engraved with MedicAlert + Safe Return’s 24-Hour emergency response number
• Live 24-hour emergency response service for wandering and medical emergencies
• 24-hour family notification service during wandering incidents and medical emergencies
• 24-hour relaying of key medical information to emergency responders
• 24-hour care consultation services provided by master's level counselors

Add $35 for caregiver ID Jewelry and membership OPTIONAL

• Membership includes everything listed above
• The caregiver wears this worldwide recognized ID jewelry to alert others that he or she provides care for a MedicAlert + Safe Return member, in case of an emergency
  

$35 annual renewal fee

• This fee helps MedicAlert + Safe Return respond to the thousands of calls for help each year
• Please note that failure to renew does not affect service if the enrollee wanders.
  Payment of the annual fee is required to add or update your Personal Health Record.

Registration for the Program

The New York City Chapter wants to ensure that everyone who needs the protection of the MedicAlert + Safe Return Program is enrolled, regardless of their ability to pay.

Registration can be done in the following ways:

By mail: Please complete a registration form (you can get the form at our chapter office, your nearest police
precinct, print one out ( PDF), or send for one via e-mail to: Elizabeth Santiago and mail it with your payment and a photo of the registrant to the address indicated on the form.
