The Dementia Support Services Program promotes aging in place throughout the Finger Lakes DDSO region with great sensitivity for the health and safety of persons with developmental disabilities (DD) and Alzheimer's disease and other dementias (ADD). This model uses an approach that blends support and management strategies to delay or avoid nursing home placement by increasing collaboration and cross-agency responses to the home and community-based needs of persons with DD and ADD and their caregivers.
