Grant and Richland counties and Columbia/Sauk counties receive Alzheimer’s Association “Healthier Wisconsin” Partnership Assistance
Grant and Richland counties and Sauk and Columbia counties are now part of a statewide initiative to improve rural dementia health through a community-academic partnership funded by the Medical College of Wisconsin and the Helen Bader Foundation. Suzanne Bottum-Jones, a nurse and health educator living on a family farm in Montfort, and Liz Miller, who has Master’s Degree in Administration and Guidance and Counseling and lives on a 17 acre farm in Sauk County, have been hired by the Alzheimer’s Association, South Central Wisconsin Chapter to work with the local medical community and local support services to assist people with dementia and their families dealing with the many challenges caused by all forms of dementia, including Alzheimer’s disease.
“As the Baby Boom generation ages, it’s important that we take steps now to address the projected increase in Wisconsinites with dementia,” said Daniel Bader, president of the Milwaukee-based Helen Bader Foundation. “We are pleased to work with the Medical College and the Alzheimer’s Association to ensure that more families can receive the assistance they need, right in their home communities.”
The three year grant provides for 4.5 new outreach specialists to be strategically located in rural Wisconsin communities. By working with people struggling with dementia, families, social service agencies and the medical community, the Alzheimer’s Association’s goal is to create formal connections so everyone impacted by dementia receives the medical care and support they need to deal with its challenges. Dr. Rhonda Montgomery and graduate students at the University of Wisconsin-Milwaukee will be compiling outcomes based data so the results of the project can be shared nationwide.
