Purpose:   The four Virginia chapters of the Alzheimer’s Association have joined together to seek a new Alzheimer’s awareness license plate in Virginia.  The chapters must collectively gather 400 pre-paid applications for the new colorful license plate by November 29 in order to begin the General Assembly and Department of Motor Vehicles approval and production process.  The license plate fees are $25.00 annually in addition to registration fees.  100% of the proceeds will benefit the delivery of programs and services for all Virginians affected by Alzheimer’s disease

Instructions: If you are interested in being one of the first to receive an Alzheimer’s awareness license plate please complete the Department of Motor Vehicle application by clicking here

1.  In the “Application Request” section of the form be sure to check the Vehicle License Plate box at the top.

2.  In the “Type of Plate” section, check the “Special Interest” box and insert “Alzheimer's Association” in the blank line.

3.  Complete the remainder of the form with the information requested.

4.  Make a $25.00 check out to the Alzheimer’s Association and mail the check and completed DMV application to:

5.  If approved by the General Assembly and DMV, license plates will be issued in 2010.  If for some reason the request is not approved, you will receive a full refund.

Please note that the Central and Western Virginia Chapter of the Alzheimer’s Association is acting as the central repository for applications and payments.  Once approval is granted, the applications with payment will be forwarded to DMV.
