Receiving a diagnosis of memory loss, dementia or Alzheimer’s disease is an emotional experience for both the patient and their families.
The Alzheimer’s Association can help through our Memory Loss Referral Program, specifically designed to assist medical professionals. If you have a patient with memory loss, dementia or Alzheimer’s disease, simply complete our HIPAA-compliant referral form. A local family services coordinator will contact the patient and their family within two business days to make them aware of resources, education and support services that will help them understand and cope with the disease.

The Memory Loss Referral Program:
• Will help your patient and their family understand the impact of memory loss and how they can work together to manage its effects.
• Is a timely and personal follow-up to a diagnosis.
• Connects your patients and their caregivers to needed resources, education and support services.
• Provides updates to the referring physician, if requested.
