Reach Out Support Program available for families caring for a loved one with Dementia
The "Reach out" program provides information about Alzheimer's disease, safety, caregiver health, social and emotional support, relaxation techniques and more. The program involves a series of six meetings conducted by home visits and is availale at no charge to families living in Central Vermont and the Northeast Kingdom. Click here for more information.
Know the 10 Signs: Early Detection Matters
Know the 10 Signs is a family education program that aims to increase awareness of the warning signs of Alzheimer's and the benefits of early detection and early diagnosis.
The Basics: Memory Loss, Dementia, and Alzheimers
This workshop is for anyone who would like to know more about Alzheimer's disease and related dementias. The workshop's information includes:
- Symptoms of effects of Alzheimer's disease and other types of dementia
- How Alzheimer's effects the brain
- Causes and risk factors
- How to find out if it's Alzheimer's disease
- Treatment

Living with Alzheimer's: For People with Alzheimers
This program is an interactive three-part program where you will have a chance to hear from others who have been where you are. We will discuss what you need to know, what you need to plan and what you can do to navigate this chapter of your life.
This program will cover information for people with a diagnosis of Alzheimer's disase or related dementia. Topics covered will include:
- Understanding Alzheimer's disease and memory loss
- Partnerng with your medical team
- Effective communication and telling others about the diagnosis
- Coping with changes and making decisions
- Daily strategies and safety issues
- Opportunities for supporting research
- Resources for people with early-stage Alzheimer's disease
Living with Alzheimers for Younger-Onset Alzheimer's
This is a series of education programs that provide answers to the questions that arise for people concerned about Younger-Onset Alzheimer's disease or a related dementia. Hear from those directly affected and learn what you need to know, what you need to plan and what you can do to ease the impact throughout the course of the disease.
This program will cover information for families dealing with younger-onset Alzheimer's disease or a related dementia. Topics covered will include:
- Developing a care team
- Legal and financial planning
- Coping strategies
- Care and treatment options
- Communicating and connecting
- Maximizing independence

This series of education programs provide answers to the questions that arise in the early, middle, and late states of the disease. Hear from those directly affected and learn what you need to know, what you need to plan and what you can do at each point along the way.

Legal and Financial Planning for Alzheimer's Disease
This interactive two-part program gives you the chance to learn about the important legal and financial issues to consider, how to put plans in place, and how to access legal and financial resources near you.

- Legal documents you'll need and wha they mean for all of you
- How to find legal and financial assistance
- Practical strategies for making long-term plan of care
- Tax deductions and credits
- Government programs that can help pay for care

The Namaste Room at Helen Porter Healthcare & Rehab Center...is a memory care program designed to meet the changing needs of individuals as they experiece the progression of symptoms and increase of cognitive loss associated with a diagnosis of dementia. The Namaste Care Program is designed to meet the physical, emotional and spiritual needs of those in the advanced stages of dementia through loving touch, meaningful activities and the presence of others. "Memory Tuesdays" at Helen Porter Healthcare & Rehab Center.... Their newest program offering free, private memory screenings to the public. A health care provider may refer or an individual can self-refer. A 10 minute cognitive screening tool is used to assist in the detection of mind cognitive impairment (MCI). The screening assesses attention and concentration, executive function, memory, language, visuo-constructional skills, conceptual thinking, calculations and orientation.
