Community Resource Fair
A fair will be offered at both sessions from 5 – 6:30 p.m. with information on a variety of services including home health services, financial planning, nursing homes and many more.

Registration Information & Conference Fee
The cost is $10.00 per person for a single session or $15.00 to attend both.  Pizza, salad and pop are included.

Please pre-register for the conference by Friday, April 3^rd, 2009. To register you may do one of the following:

1) Call the Alzheimer’s Association at (716) 626-0600 / (800) 272-3900.

2) Open the Conference Brochure Document and fill out the registration form and either mail or fax it to:

Respite Care at Conference
