The Knight Family Dementia Care Coordination (DCC) Initiative is a partnership between the Alzheimer's Association and participating health systems to improve care for people concerned about memory loss, or those recently diagnosed with Alzheimer’s or other dementias. The initiative connects individuals and their families to free Association resources and helps them to create a customized action plan. 

The DCC Initiative is currently available at a growing number of health systems in Illinois, Missouri and Washington. Talk to your doctor to see if your health system is participating in DCC. If it is not yet available, you can still access the same services and support. Contact our helpline at 800.272.3900 at any time to speak to a master's-level clinician and learn about dementia and Alzheimer's resources for people living with Alzheimer's or dementia and their caregivers.
