Care Counselor, Part Time - Monroe County MI

General Summary

The Specialist, Care Consultant will implement the Michigan Dementia Care and Support Pilot Program, which strives to support individuals with dementia and their caregivers in Monroe County. This position is responsible for conducting home visits to individuals and families in order to provide assessment, counseling and care management services. In addition, the position is responsible for assisting in the evaluation and development of the program. The position reports to the Program Director or designee and is  20 hours a week.

Essential Job Functions

  • Conduct home visits and written individual and family assessments
  • Develop person centered care plans
  • Provide individual and family counseling
  • Provide care management services
  • Assist clients in accessing services and resources
  • Maintain client files and prepare reports
  • Conduct program evaluations
  • Develop and update relevant printed materials
  • Prepare and conduct community presentations to expand community awareness of Alzheimer's disease, other dementias and the Alzheimer's Associations' programs and services
  • Maintain knowledge and referral base through networking with other professionals
  • Represent the Alzheimer's Association at public forums, conferences, and media events; function as an advocate representing the Alzheimer's Association in the community
  • Assist in the marketing of agency programs and services
  • Assist supervisor as needed
  • Minimum Requirements

  • Master's degree in Social Work or related field
  • Master's of Social Worker License in the state of Michigan (LLMSW or LMSW) preferred
  • Minimum of 2 years experience in Counseling preferred
  • Experience with home visits preferred
  • Working knowledge of community resources and Alzheimer's disease and other dementias preferred
  • Ability to effectively communicate in writing, verbally and electronically
  • Proficiency with Microsoft Office applications and Google Suite (mail, calendar, drive)
  • Must possess a reliable personal vehicle, valid driver's license and proof of valid insurance.
  • Ability and willingness to travel as needed to conduct Chapter business.
