Social Worker or Social Worker/Union County Coordinator

General Summary

The Alzheimer's Association, Central Ohio Chapter provides family

support and education to those affected by Alzheimer's disease and other dementias.

The following full-time/part-time opportunity exists for a social worker who is interested in being part of our interdisciplinary team.

Essential Job Functions

Social Worker:

This position assists families in creating goals and formulating action plans. This position can be combined to include programs and services to Union County residents (see below).

The social worker is responsible for conducting bio-psychosocial assessments with families in the office and home settings that focus on safety concerns, medical issues, future planning, community resources, caregiver education, and caregiver stress for those affected by Alzheimer's disease.

Social Worker/Union County Coordinator:

The Alzheimer's Association, Central Ohio Chapter provides education and support to Union County residents, 16 hours per week. This includes providing educational programming for caregivers, coordinating a weekly social engagement program for individuals living with dementia, and co-leading a

monthly support group. This would also include a yearly provision of an educational symposium for Union County.

Minimum Requirements

Master’s degree in Social Work with current license preferred, but a Bachelor’s degree in Social Work with extensive experience in the field of aging will be considered.
