Care Consultant, Part Time (MI)

General Summary

The Care Consultant will implement clinical services, including Information and Referral and Care Consultation services to constituents residing within the West Shore region of MGLC.  The employee in this position is also responsible for the oversight of the Chapter's involvement with national safety services via the Common Program Portfolio (MASR/Comfort Zone).  This position may operate off site several times each month to offer walk-in care consultation service via "Answers on Alzheimer's".  The position reports to the Program Director or designee.

This is part time (16hrs per week) non-benefits eligible position.

Essential Job Functions

  • Conduct Information and Referral events and/or Care Consultation events as indicated by constituent need, on and off site
  • Provide individual and family counseling and care management services
  • Assist clients in accessing services and resources
  • Complete required training and practice for persona assignment- Personify 360 (CRM)
  • Conduct program evaluations
  • May provide clinical supervision to junior staff and supervision to office volunteers
  • Develop and update relevant printed materials
  • Prepare and conduct community presentations to expand community awareness of Alzheimer's disease, other dementias and the Alzheimer's Associations' programs and services.
  • Maintain knowledge and referral base through networking with other professionals.
  •  Represent the Alzheimer's Association at public forums, conferences, and media events; function as an advocate representing the Alzheimer's Association in the community
  • Recruit, train, retain and oversee volunteer support group facilitators.
  •  Assist in the marketing of agency programs and services.
  • Assist supervisor as needed.

Minimum Requirements

  • Master's degree in Social Work or related field, or commensurate experience.
  • Master's of Social Worker License in the state of Michigan (LLMSW or LMSW) preferred.
  • Minimum of 2 years experience in Counseling preferred.
  • Experience with home visits preferred.
  • Working knowledge of community resources and Alzheimer's disease and other dementias preferred.
  • Ability to effectively communicate in writing, verbally and electronically
  • Proficiency with Microsoft Office applications and Google Suite (mail, calendar, drive)
  • Must possess a reliable personal vehicle, valid driver's license and proof of valid insurance.
  • Ability and willingness to travel as needed to conduct Chapter business.
