The Alzheimer’s Association – Greater Michigan Chapter has a number of Care Counseling Programs are designed to meet the needs of individuals with memory loss and their care partners:

  • The Care Consultation - Level 1 Program is a short term program, designed to meet the needs of individuals with memory loss and their caregivers, provides a verbal plan of action, support and community resources via in person or over the phone.
  • The Care Consultation - Level 2 Program is designed to meet the needs of individuals with memory loss and their care partners where they are. It includes an assessment of needs/desires, the creation of a person centered plan of care, coordination of appropriate resources, as well as ongoing counseling and support. This program is available over the phone throughout our service area and also in-person in some areas.
  • The Integrated Care Counseling Program is a health care partnership for the initial diagnosis, brain health coaching and counseling/care management of persons with memory loss and their care partners. The program pairs a social worker from the Alzheimer’s Association – Greater Michigan Chapter with a hospital neurologist or diagnosing physician to aid in the diagnosis of dementia and assist with ongoing treatment and future planning. Currently, we have partnered with the Henry Ford Health System in Metro Detroit and Marquette Internal Medicine and Pediatrics Associates in Marquette. 
  • The Wraparound Program is a comprehensive, in-home care consultation program designed to meet the needs of individuals with difficult to manage behaviors, wraps services around those with memory loss and their families. The program works with a Community Collaborative Team, a collaboration of agencies specializing in mental health, public health and aging services, to help families navigate through the disease process.
