Statewide Care Consultations

If you or someone you know is living with dementia or has memory concerns, the Care Consultation program may be the right fit for you. A counselor will work with you to discuss your needs and come up with a plan to meet them.

This short-term program is offered by phone statewide and can take place in our office or in your home depending on your location.

West Michigan Dementia Support Program

The Dementia Support Program provides assistance to individuals and families caring for someone with dementia in Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Newaygo or Osceola counties. If you are caring for someone with memory loss at home and you or your loved one is aged 60 or older, you may be eligible for the Dementia Support Program. When enrolled, one of our licensed social workers will come to your home to help address caregiving concerns and behaviors as well as create a customized care plan, provide dementia education and connect you with community resources. Integrated Care Counseling Program

This program is a health care partnership for the initial diagnosis, brain health coaching and counseling/care management of persons with memory loss and their care partners. The program pairs a social worker from the Alzheimer’s Association Michigan Chapter with one of our partner healthcare providers to aid in the diagnosis of dementia and assist with ongoing treatment and future planning.
