Care Consultation is a service that can help you and your family develop a road map to navigate through the many thoughts, emotions, and questions you may have about memory loss and dementia. Care Consultation can be useful whether you or someone you love has just been diagnosed, or if you and your family are seeing things from different perspectives and perhaps feeling overwhelmed. You will receive valuable one-to-one assistance that will enable you to better understand the disease, manage the symptoms of dementia and cope better with the disease.
Care Consultations (both phone and in-person) will include:
Identifying your current needs
Assistance with developing a plan
Assistance with finding resources and services
Providing education and support
Providing ongoing support and follow-up
