Introducing the Dementia Care Planning Toolkit

The purpose of the toolkit is to facilitate the development of person-centered dementia care plans that support living well with dementia. Its target audience includes people living with dementia, caregivers, families, community-based service providers, and healthcare providers.
Two webinars have been scheduled to review and discuss the toolkit. To make sure the conversation is most relevant to you, each is tailored to a different audience. There is no fee to participate.
- Thursday, March 20 | 12:00 - 12:40 PM - This webinar is designed for people caring for family or friends living with dementia or those who live with dementia themselves.
- Thursday, March 27 | 12:00 - 12:40 PM - This webinar is designed for community-based service providers and health care providers.
