A diagnosis of Alzheimer's or another dementia is life-changing. The Alzheimer's Association Dementia Care Coordination program offers a variety of resources to help your patients and their families live their best lives. Caregivers referred to the program will receive a care consultation and individualized care planning. Our clinical team will provide a summary report back to you.

All of our Dementia Care Consultants have experience working with people living with dementia and their caregivers. They have all been trained by the Alzheimer’s Association to provide a high level of support and education to the caregivers they work with.

We are excited to be able to offer Spanish and Haitian Creole language care consultations by our bilingual Dementia Care consultants. We are also able to provide a high level of support for other non-English speaking caregivers through the use of our language line that offers translation in over 200 languages.

Diagnosis of a specific type of dementia is not required to participate in the program.
