The Alzheimer’s Association - Western Carolina Chapter is here to work together with physicians and other healthcare professionals to ensure that individuals diagnosed with Alzheimer’s disease or related dementias receive the appropriate care and resources needed to improve their quality of life.  Together we provide a holistic approach to patient care.

Direct Connect Referral Program
 
Direct Connect Referral is an easy way to connect your patients to the Alzheimer’s Association - Western Carolina Chapter.  This free service offers resources and support to help caregivers and their loved ones navigate the challenges with memory loss, Alzheimer’s disease and other dementias.  Simply complete the one-page, HIPAA compliant form and fax or scan it to our offices.  Once received, the Western Carolina Chapter will contact the referred individual within 3 business days and assist with immediate needs.

Physician Toolkit
 
The Physician Toolkit provides Alzheimer’s Association resources that physicians and other healthcare professionals can use to talk with their patients, regardless of the setting or their familiarity with the Association.

    Alzheimer’s Disease Pocketcard Toolkit
     
    Get reliable dementia information at your fingertips.  Accessible through a mobile app and online portal, the Alzheimer’s Disease Pocketcard helps the entire clinical team manage Alzheimer’s disease care with confidence.  Key features of the Alzheimer’s Disease Pocketcard include:

  • Clinical information on the diagnosis and management of Alzheimer’s disease and other dementias.  Interactive tools to access cognition, depression and functional status. 
  • Education packets, individualized by disease stage that can be printed out or emailed directly to patients and caregivers.  Includes a packet in Spanish.
  • Professional resources including free CME credits for Alzheimer’s-related topics, instructional videos and guidelines and recommendations.

  • Packet of Hope
     
    Packet of Hope is an informative resource for recently diagnosed patients and their families.  Physicians can provide this packet as a tool to help assist families in dealing with Alzheimer’s and other dementia concerns.


     
    Minds in Motion Early-Stage Program
     
    Minds in Motion Early-Stage Program is a 7 month program for individuals living in the early stage of Alzheimer’ disease or related dementias and their care partners.  Participants enjoy group activities including, education programs & events, weekly support groups, & social engagement activities.
