Governor McMaster has proclaimed April to be South Carolina Health Care Decisions Month, a time to inspire, educate and empower citizens of all ages to take control of their future health care choices. View the proclamation.

Especially since the beginning of the COVID-19 pandemic, too many people have been faced with the sudden need to consider what health care they (or their loved ones) would want, should they be confronted with a critical health care emergency. We never know when an unexpected health care event might occur, but we can take steps now to be prepared.

For those facing Alzheimer's or related dementia, advance care planning is even more important. Because of the progressive nature of dementia, individuals in the early stages should take care to express their wishes now, to discuss those wishes with their family members, and to make sure those wishes are written down to ensure they are followed when appropriate.

Discussing advance care plans with your family and care team can be difficult and emotional. However, there are lots of tools to help you consider and record future care options in order to share them with family members, your care team or a legal professional.

Start planning today:
  • Check out this easy-to-use guide, provided by My Life My Choices, South Carolina's Advance Care Planning initiative.
  • Learn more about SC POST, a standardized medical order that allows patients with a serious illness to choose which medical treatment(s) they want to receive and identify those they do not. This is a portable document that transfers with the patient between medical settings.
  • Attend one of our free virtual classes on Legal and Financial Planning (April 14 at 12:00 PM) or Legal Planning (April 28 at 10:00 AM). Can't make these times? Take the class on demand in our free online Training and Education Center.
  • Browse our web resources on end-of-life planning.
