Medical and Healthcare Professionals

Giving or getting a diagnosis of Alzheimer's disease or dementia is not easy. Patients and caregivers have many questions that continue from the onset of diagnosis through the progression of the disease, especially as new challenges arise. We know addressing these issues takes time and we understand that the constraints of a busy healthcare setting can potentially put restrictions on the support necessary to navigate the uncertainties and non-medical difficulties that face patients, caregivers and family members at every stage of the disease.

  The Alzheimer's Association WNY Chapter works to build relationships with healthcare professionals by providing education about dementia and facilitating patient and caregiver connections to free supportive services offered by the Chapter.

Rapid Referral Program

The Rapid Referral Program provides an opportunity for you to link families directly to a WNY Chapter care consultant by completing a simple one page form and emailing or faxing it directly to the Chapter Office. After we receive it, we will contact the individual you've referred to us within two business days and assist them with immediate needs, while offering continuing assistance as needs change and challenges progress.
