Background

Individuals with Alzheimer's and other dementia often require the services such as  hospitals and other long-term care facilities, especially as the disease progresses. Studies have found that many individuals living with dementia often prefer to “age in place” or live independently in one’s own home and community. This can foster a sense of belonging and improve overall quality of life. However, there is a need to develop new approaches to support aging in place care for individuals living with dementia.

Research Plan

Dr. Mayan and colleagues will examine the feasibility of the “The Return Home”, an Israeli Ministry of Health initiative that will transition 100 older individuals living in long-term care facilities to their homes where they will receive medical and social care. A mobile phone application will be used for communication and help foster support between caregivers and the medical team for each individual living with dementia. The study will be conducted over a 12-month period after which the team will evaluate the impact of the home care and use of the application on the participants’ cognitive function and their caregivers’ quality of life.

Impact

The findings from this study may highlight the relationship between community and living environment with quality of life and may provide new insight into long-term care options for individuals with dementia.
