Direct Connect Rapid Referral
It takes time to talk about dementia. We have the time. Direct Connect Rapid Referral provides free support for families facing all types of dementia, including:
- Education regarding dementia symptoms, disease stages and care tips.
- Connections to resources in the community.
- Support groups and activity programs.
- Personal support to help families effectively plan, cope and manage everyday challenges.
- Early-stage programs designed especially for newly-diagnosed individuals and their families.
- Connection to local and national clinical research trials.
