The Knight Family Dementia Care Coordination (DCC) Initiative is a partnership between the Alzheimer's Association and participating health systems to improve care for people concerned about memory loss, or those recently diagnosed with Alzheimer’s or other dementias. The initiative connects individuals and their families to free Association resources and helps them to create a customized care plan.
There are many medical, financial and personal benefits to getting an early diagnosis.
How the program works
Are you or someone you know experiencing problems with memory or thinking? The first step is to talk to your doctor.
If your doctor thinks that you or your caregiver might benefit from the program, they can refer you to the Alzheimer's Association to schedule a free care consultation.
As part of the consultation, trained and knowledgeable staff will contact you to discuss your and your care partners' unique needs.
Your customized consultation
The consultation will be customized to your needs and situation, including but not limited to topics like:
- What you need to know about Alzheimer's.
- Communication techniques.
- Living with Alzheimer's.
- Long-term planning.
- Information about resources available from your local Association chapter, such as support groups, social programs for people living with early-stage Alzheimer's and dementia, and educational sessions.
At the end of the conversation with the care consultant, the care partner will receive a personalized action plan for supporting the person living with Alzheimer's.
