The Alzheimer’s Association of Colorado has been recognized as a Community Quality Champion for its efforts to help make healthcare safer, more accessible and cost-effective.Quality-Seal-FNL.png

The Association was recognized by Telligen QI Connect. Telligen works with the Centers for Medicare & Medicaid Services (CMS) to implement key elements of the Department of Health and Human Services’ National Quality Strategy and federal healthcare reform efforts. Telligen has been active in Colorado, Illinois, Iowa and Oklahoma for the past 45 years.

“Our work with Telligen is a pivotal part of our work with the healthcare community in Colorado and the Rocky Mountain region,” said Danelle Hubbard, director of Health Systems for the Alzheimer’s Association in the Rocky Mountain region. “Many of Telligen’s priorities, such as reducing hospital readmissions, align with our work in early detection and diagnosis to reduce or eliminate unnecessary hospitalization for the Alzheimer’s population.”

A commitment to the Colorado healthcare community
Under Hubbard’s direction, the Alzheimer’s Association has engaged with health systems across the state and region to actively participate in the Colorado State (Healthcare) Plan to represent the 76,000 Coloradans living with Alzheimer’s disease in terms of the state’s decision-making process, identify resource needs, and address health equity in Colorado.

“We know that Alzheimer’s disease and dementia are patient care disruptors costing both patients and providers time, money and quality care,” said Hubbard. “However, there are proven methods to minimize or even eliminate these negative outcomes. The goal of the Alzheimer’s Association’s Health Systems Initiative is to support providers with strategies and solutions that can ensure quality care for people living with dementia and better understand the needs of providers so we can help them meet the complex care needs of individuals and families.”

The Alzheimer’s Association’s Health Systems Initiative is in its third year and the recognition by Telligen is an indication that it is working.

“We are seeing great progress, establishing relationships with the largest health systems across Colorado and the U.S., and making a meaningful impact on the lives of Coloradans seeking care,” said Jeff Bird, executive director of the Alzheimer’s Association of Colorado. “There’s much work to be done, but we can measure the progress.”
