    Giving Through Beneficiary Designation

    Naming the Alzheimer's Association as a beneficiary of your bank, brokerage or other financial accounts is a simple and effective way to support our mission to end Alzheimer's and advance research, care and support. These donations pass directly to us, avoiding probate and potentially reducing estate taxes.

    Benefits of giving by beneficiary designation

    You retain control of your assets during your lifetime. You can change your beneficiaries at any time.

    Consider naming the Alzheimer's Association as a beneficiary of the accounts below and leaving lesser-taxed assets to your heirs, as some of these accounts may be taxed at high rates:

    • Individual retirement accounts.
    • Life insurance policies.
    • Bank or brokerage accounts.
    • Certificates of deposit.
    • Donor-advised funds.

    You can leave a gift of any amount or a percentage, and no minimum contribution is required.

    To designate the Alzheimer's Association as a beneficiary, contact your financial institution or plan administrator and request their beneficiary designation form. Complete and return the form.

    You can honor the memory of a loved one through your gift. Your gift will accelerate research, help provide care for those with Alzheimer's, and move us closer to a cure.

    How a beneficiary designation works

    • Contact your retirement plan administrator, insurance company, bank or other financial institution for a beneficiary form.
    • Decide what percentage or amount you would like the Alzheimer's Association to receive and name us, along with the percentage you chose, on the beneficiary form.
    • Use Tax ID #13-3039601. Our legal name is the Alzheimer's Disease and Related Disorders Association Inc. dba The Alzheimer's Association.
    • Return your completed and signed form to your plan administrator, insurance company, bank or other financial institution.
